Direct Primary Care. An innovative model of healthcare that puts the caring back into medicine.

Megan L. Miller APRN CNP

“I am passionate about bringing patient-centered care back to my community while also providing each patient with holistic, accessible, no hassle healthcare.”

Working as a Certified Family Nurse Practitioner, Megan found downfalls and weak spots in our healthcare system. The main problem? Patients lack a “healthcare home,” somewhere you and your family can always call and someone will always answer. This inspired her to offer Direct Primary Care to her hometown, Springfield, Ohio.

Megan graduated from Shawnee High School and then pursued her degree and certifications through The Ohio State University and University of Cincinnati. Megan is furthering her certification in Functional Medicine and involved in an Anti-Aging Fellowship.

Megan moonlights for Dayton Center for Neurological Disorders where she spent the majority of her nurse practitioner career and is a Senior Director with a clean beauty brand, BeautyCounter.

Megan has a passion for the Springfield community, avidly involved in local non-profits and various local organizations. She loves staying active and exercising at Springfield Health & Fitness. Not only is Megan a self-proclaimed foodie, but she also enjoys a dirty martini from time to time.

“I am passionate about working collaboratively with patients of all ages to develop healthy lifestyles, promote health and prevent disease.”

Leigh Ann Swank, APRN-CNP

Leigh Ann graduated with her Associates Degree of Nursing from Kettering College of Medical Arts in 2008. She worked for Premier Health for 8.5 years in Advanced Care, Emergency Department and PACU. In 2016 she completed her master’s degree at Ohio University and was certified as a Family Nurse Practitioner. She spent the last 7 years practicing Family Medicine in Champaign County, Ohio.

During that time she discovered a lack of available resources for patient’s struggling with their mental health. She obtained her Post Master’s Certification as a Psychiatric Nurse Practitioner in 2020 through the University of Cincinnati. Leigh Ann was drawn to the Direct Primary Care Model because it allows patients greater access to personalized care. The model allows her to spend more time working with her patients to formulate individualized health goals to improve their overall quality of life rather than meeting the goals of large healthcare systems.

Leigh Ann lives in Champaign County.  She and her husband have been married for 24 years and they have two children. Leigh Ann enjoys spending time with family and friends, attending sporting events and reading.

In 2014 Susan obtained her Family Nurse Practitioner and was eventually recruited for psychiatry due to her use of pharmacogenomics. Now obtaining her certificate for Psychiatric Mental Health Nurse Practitioner, she will be board certified in two specialties - Family Practice and Psychiatry.

Susan loves working with patients who have neuropsychiatric issues to help them attain a great quality of life.

Susan has been married for 44 years! She is the proud mother of 7 children and 9 grandchildren! She loves learning new things and enjoys classical music.
