If you're not feeling well and you're a Tier 1 through Tier 4 member, please contact our office as soon as possible so we can help guide your next steps.
When reaching out, be sure to include:
How long you’ve had symptoms
A full list of symptoms you're experiencing
What treatments or remedies you’ve already tried
What has helped (or not helped)
Any known exposure to illness
What you're hoping to gain from the visit (e.g., diagnosis, treatment, reassurance)
Providing this information upfront allows us to care for you more effectively and determine if an acute appointment is needed.
